Completed Personal Particulars Form 
Six (6) clear photos (size 51 mm by 51 mm, with a white background and dark wear)
Current Passport with a clear photocopies of its first and last
Address pages, valid UAE visa page and any other endorsement pages
A photocopy of a photo ID card with expiration details (such as a Driver’s License, Emirates ID Card, Labor Card, or Health Card).
Original passport of the minor applicant
Need valid Emirates ID, Original passport of the minor applicant.
Both parents of minor applicant must have original passports and Emirates ID for verification.

For new employees

A valid original and photocopy Employment Paper Visa, a Photo Testified Letter from the company and a valid Trade license copy.

For passports that expired for more than 1 year

Prior approval a request letter that contains an explanation from the Consulate General of India and Instruction Note that provided by BLS International
